Miami plastic surgeon Constantino Mendieta, M.D. says more local women – from Dania Beach to Biscayne Bay and from Surfside to Hialeah Gardens – are asking about butt augmentation. They want to go from flat or sparse buttocks to round, curvy, spectacular or, as Dr. Mendieta says, "bootylicious."
Buttock augmentation has been around for many years although few have heard of it until the last several years. Thus, only a handful of U.S. surgeons are thoroughly schooled in the procedure and offer it to patients. However, Dr. Mendieta studied in Brazil where the emphasis has been placed for many years on the graceful contours of the female derriere. He learned the craft of buttock augmentation from pioneer Brazilian surgeons who still create some of the most shapely, attractive female rears ever. The most typical patient for a buttock augmentation comes to see Dr. Mendieta because she has a boxy, flat or square rear end and wants more curves. Or, she may be very thin and just wants more volume in her rear.
There are two butt enhancement methods: fat transfer (“Brazilian Butt Lift”) or buttock implants. Based on your skin tone, the amount of fat in other areas of the body, and desired results, Dr. Mendieta will recommend the appropriate method. If you are very thin, and do not want implants, Dr. Mendieta may ask you to gain some weight and return. However, if you have fat to spare, the peaks of your buttocks can be made more substantial and curvier by injecting some of your own fat.
Usually, if a patient is overweight or wears an eight to 12 dress size, she is probably best suited for transfer and perhaps some liposuction. Thin patients who wear size two to four dresses are usually not good candidates for fat transfer because they do not have any fat to spare.
Dr. Mendieta offers his signature "Highlight Liposculpting and Lipo Fat Transfer" 3D approach, which is designed to accentuate the patient's own curves. When performing butt augmentation through fat transfer, patients will receive the added benefit of undergoing liposuction ("lipoplasty") on one or more areas of the body. Ideal candidates for butt enlargement with fat transfer are typically between a size eight and 12.
Augmenting your derriere with your own body fat is very appealing – the results can look very natural and, because fat is your own body substance, there is little chance for rejection. But, for all the problems involved in losing weight, fat can be hard for surgeons to work with unless the doctor and his staff have had a lot of extra training and experience working with it, such is the case of Dr. Mendieta and his staff. Dr. Mendieta creates shapely female bottoms through fat grafting by taking corpulent flesh from heavier areas of the body (i.e., flanks, outer legs, upper and outer buttocks) and injecting it onto the peaks of the buttocks to create one shapely rear end with much more curve.
Fat has some interesting properties. If it is injected into muscles or bones, it feels like muscles or bones once it’s under the skin. Likewise, if fat is injected into a more corpulent area it feels like, well, fat. The downside of transferring fat to the buttocks, face, lips or any other place in the body, is that some of it will be reabsorbed by the body. And often, surgeons can’t really predict how much will be reabsorbed. What we can say is: what you have remaining after three months will be yours permanently.
This fat is harvested and injected into the buttocks to produce enhanced buttocks. Dr. Mendieta prefers to inject the fat in pearl-sized drops into different layers of the muscles and fat, starting with the deepest layer. By doing so, the fat cells are closer to a blood supply and are more likely to survive the transfer. For butt augmentation, the layering method will create a smooth, more rounded buttock with no irregular places showing through the skin.
Of course, every Brazilian Butt Lift case is different, but the amount of fat required for each buttock is an average of about 250 to 350 cubic centimeters (cc) of pure fat. So, for both sides, that equals about 700 cc which is about ½ to about ¾ of a pound of fat. Most people are very pleased to have that amount of fat removed from their bodies. If you can spare the fat, then fat transfer is the best option; however, if you are very thin and carry very little body fat, you may be best served by having an implant inserted into each of your buttocks. According to our office records, the fat “takes” 70 to 80 percent of the time, given correct harvest and reimplantation procedures.
Almost all rear ends look better with some added volume. Even after doing hundreds of fat transfers, Dr. Mendieta is always amazed at how much nicer the gluteus looks when more volume is added. And remember, it is not always a matter of how much fat is contained in one area but where the fat is located that actually decides what the shape of buttocks will be. Dr. Mendieta usually suggests gluteal augmentation via the fat transfer method to most patients who are interested in having their buttocks shaped and contoured.
You may find some myths about fat while you are researching the procedure. One of the most widespread is that fat transfer will result in a lumpy or bumpy surface. So after fat transfer into your buttocks, your skin and rear will feel very natural and appear like it is your own – which it is because the fat has incorporated into your body.
Keep in mind: Just after the procedure, the buttocks will swell somewhat and then subside. Thus, most patients think all the transferred fat has gone away but actually, it was the swelling that went down. That’s why it requires three months before we can get a final resolution on how much fat actually remains.
Most patients want to know right at first how long it takes to recover from a fat transfer into the buttocks. Our yardstick for that is the length of time it takes for you to recuperate from the liposuctioned areas. The liposuction incisions are very small, just enough to allow the suction wand, or cannula, to go under the skin and take out the fat that we work with. However, your buttocks won't feel any discomfort at all. After the procedure, you must wear a body garment for six to eight weeks. You can go back to work after 10 to 14 days.
